 Post subject: Engineered Hardwood turns green

I am an installer. I had trouble with an engineered product, crunching noise when you walk. I removed the hardwood floor. The middle layer of the wood had turned green, and was swollen. What can cause this? The floor was stapled, on plywood, over felt paper.
