I've started to re-consider gluing a floor to my concrete slab and just go with a floating. I'm still concerned about the hollow-type noise. What are my underlayment choices for sound control? Do any of them combine moisture control as well, or should I use a seperate 6 mil sheeting? TKS.

I don't trust the 2-n-1, cushion and moisture barrier combination. I have seen them fail the perm test.
